Introduction. Acute lymphoblastic leukemia (ALL) is a heterogeneous disease distinguished by clonal replication and piling of immature lymphoid cells in the bone marrow lymph organs. The etiology unknown but radiation some chemical exposure, as well genetics, might play role. onset abrupt. Clinical presentation characterized variety general symptoms: fatigue, malaise, night sweats, weight loss, fever. diagnosis based on patient's history, physical examination, blood tests, biopsy, cytogenetic, immunohistochemical tests. Core treatments are poly-chemotherapy, therapy, hematopoietic stem cell transplantation. Case report. We presented 20-year-old patient. On his first visit, he complained neck pain. He was treated physiotherapist. After finished with therapy we noticed pancytopenia lab work. His practitioner (GP) referred him to hematologist where further medical examinations were performed ALL made. treatment started chemotherapy regimen. An allogeneic transplantation afterward. In follow-up, regular haematologic assessments done, as, assessment MRD (Minimal Residual Disease). Complete morphologic remission maintained whole time positive findings. received an antiCD22 monoclonal antibody therapeutic response good. Eight months later B-ALL relapse confirmed. condition got worse spite intensive patient died. Conclusion. whose outcome depends many factor. Due atypical symptoms, it should be taken into consideration shorten diagnosis, provide timely thus influence outcome.
